Tips for Form DS-156
- Please ensure that all fields are completed. If you have no response for a question, put “none” or “nil”.
Do not draw a line or write “NA” or “not applicable”. - All applicants above the age of 14 must sign their application form. Where applicant is below 14, either parent can sign the application form.
- The photo affixed must be front facing, not more than 6 months old with a light color background.
- Applicable if you have filled the form online while taking an appointment:For Boxes 1 through 14 in DS-156 form, you may amend, at any time, in pen if the details of your application change or you made a mistake. Do not complete and print another DS-156 Form. Amend your original forms instead.
- All applicants are personally responsible and accountable for the answers on their application forms,
regardless of who fills out the form.
